Code filter and URL filter are conflicting

Description

see http://www.projectpier.org/node/387 for an example. The url is cut short and provided as a link. This might be resolved by providing an input option that doesn't have the URL filter enabled

Also review additional/alternative setup changes to the input filters.

Fixed some of the ordering of the input filters as per this thread http://drupal.org/node/190751#comment-649651

And syntax highlighting (for php) now seems to work, plus I think i resolved some of the other conflicts.
